Applications must be submitted online through the Center for Scholarship Administration (CSA) website. There are three steps to complete the application:

1. Click the "create application" button below to answer the prequalifying questions. When you submit these, it will automatically take you to the profile page.
2. Create or update your profile. Save and continue to the online application.
3. Complete the online application. A confirmation email will be sent when the online portion of your application is complete. Applicant should contact CSA if a confirmation email is not received.
4. Mail in, or email, the packet of additional required materials. See "What constitutes a complete application" below.

Applicants Must:
• Be seniors or a graduate OR have a GED within the State of Kentucky or Indiana.
• Be pursuing a degree in the field of cosmetology.
• Be pursuing a career path as a salon owner/stylist.
• Possess traits of high moral standards, sincerity, industry, thrift, truthfulness, kindness and humility.
• Attend any accredited cosmetology school within the State of Kentucky or Indiana.
• Be enrolled full-time.
• Demonstrate financial need.

Additional Desired Qualifications:
• A minimum cumulative GPA of 2.5 on a 4.0 scale.

• Official transcript with grades and cumulative GPA posted through the latest semester.

• Signed Student Authorization and Acknowledgements Form, found in the online application, signed by the scholarship candidate.

- Submit a one-page signed personal statement explaining why you want to enter the cosmetology/hair styling business.

In order to be considered for this scholarship, please:
Submit a SIGNED copy of parents` FEDERAL TAX FORM 1040 (pages one and two only) for the latest year these forms were actually filed. In case of divorced parents, only the custodial parent`s (and current spouse's if remarried) financial information will be required. If you have already submitted your FAFSA submission summary, please submit your FEDERAL TAX FORM 1040 in order to be considered

Note: Each item should include both the applicant's name and name of the scholarship at the top of the page.

The amount of scholarship award is $2,000.

Funds may only be used to pay tuition, fees, books and supplies as required by your courses.

You must be a full time student enrolled at an accredited cosmetology school within the State of Kentucky or Indiana, in the field of study leading to a certificate within the field of cosmetology.

An eligible educational institution is one whose primary function is the presentation of formal instruction and that normally maintains a regular faculty and curriculum and normally has a regularly enrolled body of students in attendance at the place where it carries on its educational activities.

All applicants will be notified via email, whether or not they have been selected to receive a scholarship. Those students selected will download a package from their online account that includes a scholarship agreement and information about the scholarship terms.

This scholarship is for one year only.
